- Referees should still report match day discipline/misconduct to the FA through Whole Game System or the equivalent process if not using WGS but may also report direct to the League on this form.
- The League will refer reports that allege misconduct, discrimination, or raise safeguarding concerns to the appropriate County FA for investigation.
- Players/Parents/Guardians should also report any matters they may refer to the League to their Club/Team Officials and/or their Club Welfare Officer as appropriate.
- If appropriate the League may inform Club/Team Officers of the participating Team/s about the report.
