Send a report to SSYFL

  • Anyone attending a SSYFL fixture e.g. Match officials, Team Officials, Players, Spectators, may use this form to send a report to SSYFL e.g. incidents, behaviour, commendations, complaints. 
  • Reports of bad behaviour or alleging possible misconduct must be reported on this form within 7 days of the alleged offence to meet the follow-up timescales to support any discipline action that may arise. 
  • If reporting misconduct or bad behaviour read the website reporting and discipline section in Key Points.   
  • Referees should still report match day discipline/misconduct to the FA through Whole Game System or the equivalent process if not using WGS but may also report direct to the League on this form.
  • The League will refer reports that allege misconduct, discrimination, or raise safeguarding concerns to the appropriate County FA for investigation. 
  • Players/Parents/Guardians should also report any matters they may refer to the League to their Club/Team Officials and/or their Club Welfare Officer as appropriate. 
  • If appropriate the League may inform Club/Team Officers of the participating Team/s about the report.

YOUR REPORT

  • The referee fields in the form are important as the referee is responsible for match day discipline, so they will be asked about what occurred.  
  • All sections of the form must be completed to provide the League with the necessary information to consider appropriate action.  
  • Provide as much information as possible e.g. names, shirt numbers, or descriptions of any persons involved, state what was actually said or done and by whom. 
  • If your report includes information provided by another person please make this clear in your report and provide contact details, and/or ask them to complete their own report form.
